Dear Doctor*
Under separate cover today we are f o r w a ^ i n p ^ ^ o ^ a n d urine samples on our Crescentville plant employee,
This patient states that for the past five days he has been vomiting
three or foiir times a day, is suffering from anorexia, diarrhea, and
a cold. He has also stated that he lost four* pounds in the past few
days.
.:
has been working steadily for the past six months in the lasting Department. Previously to that he worked in the Shaker Departmeat at our 19th St. plant with red lead for three months. Prior to that he had been in the U. S. Army where he received a shrapnel wound in the head during combat.
In examining this employee there were no neuro muscular signs present. His last stipple count was two. How much of his symptoms are of a psycho somatic origin, the examining physician was unable to state at that time. However, it is felt that the emotional factor plays an important part in his present illness.
Kindly favor us with your report in signed duplicate together with a single bill.
Very truly yours,
FBLsRB
